Job Overview
We are currently seeking compassionate, dependable, and dedicated Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our growing home care team. In this rewarding role, you will provide skilled nursing care to clients in the comfort of their homes while helping them maintain dignity, independence, comfort, and quality of life.
This position is ideal for nurses who are passionate about delivering high-quality patient care and making a meaningful difference in the lives of seniors and individuals with chronic illnesses, disabilities, or memory-related conditions.
Responsibilities
- Provide skilled nursing services in accordance with physician orders and individualized care plans
- Perform health assessments and monitor clients’ overall condition and progress
- Administer medications, treatments, and nursing procedures as prescribed
- Assist with wound care, catheter care, injections, and other clinical interventions as needed
- Educate clients and family members on disease management, medications, and health maintenance
- Collaborate with physicians, therapists, caregivers, and other healthcare professionals to coordinate care
- Maintain accurate, timely, and compliant clinical documentation
- Observe and report changes in clients’ physical, mental, or emotional condition to the appropriate healthcare professionals
- Promote a safe, respectful, and comfortable home environment
- Ensure compliance with all applicable nursing standards, regulations, and agency policies
Supervision & Training
- Train, support, and supervise caregivers as needed on client-specific care
- Ensure caregivers follow ADL task guidelines and infection control protocols
- Conduct periodic in-home supervision visits and documentation reviews
Qualifications
- Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing required
- 2 years of nursing experience required, especially in home health, hospice, private duty, long-term care, rehabilitation, or geriatric settings
- Strong clinical assessment and critical thinking skills
- Familiarity with electronic medical records and clinical documentation practices preferred
- Excellent communication and interpersonal skills
- Compassionate, patient, and reliable with a strong commitment to quality patient care
- Ability to read, write, and understand English required
- Valid driver’s license and reliable transportation preferred
