Diabetic Management by Our Nurses
Diabetes is the 8th leading cause of death in the U.S. It is also the number 1 leading cause of kidney failure, lower limb amputations, and adult blindness.
TLC Private Home Care Nurses offer diabetic management within the assisted living facility and/or in the home.
Poor dexterity and poor eyesight make it very difficult for someone to monitor blood sugar and administer their own insulin. Our nurses can be available to take this over daily or as needed. This can allow the family peace of mind.
- Blood sugar monitoring and recording
- Insulin administration
- Ordering supplies, needles, strips, and glucometers
- Weekly skin assessment
- Diabetic foot care and ulcer detection
- Vital signs
- Nutrition teaching (only when service is provided in the home)
- Physician contact
- Documentation as requested
- All abnormalities are called directly to the physicians
Alzheimer's & Dementia Care
Day to day support for families and clients facing this most difficult disease and its challenges.
As Alzheimer's and Dementia progresses, the role of providing care for them changes as well. It can be one of the most difficult times a family may experience in their lives. TLC Private Home Care specializes in the care of patients at the end of life from 4 hours up to 24 hours a day. Our CNAs and HHAs are experienced in hospice. They are the most caring, compassionate, and empathetic individuals. All our staff will show family members grace, humanity, and kindness until the end.
